Replacement Certificates

If your OCNSWR certificate has been lost, stolen or destroyed then you may request a replacement. You should make your request through the centre where you studied who will submit a Replacement Certificate Request form to us. If this is not possible then you may request the replacement directly from OCNSWR.

Please note, to enable OCNSWR to process your replacement certificate request, a copy of your ID such as a driving license, birth certificate or passport, must be sent with your Replacement Certificate Request form.

Please download and complete the Learner Replacement Certificate Request form using the link below and send the completed form together with a copy of your ID and a cheque/postal order for £25 (per replacement certificate) to OCNSWR, Units 24/25, HQ, 237 Union Street, Plymouth, PL1 3HQ.
